GOOD SHEPHERD SERVICES

1. Workshop fee is $125.00 per person for a one-day workshop, $250.00 per person for a two-day workshop, $65.00 per person for a half-day workshop, unless otherwise noted.

2. Registration for a workshop is a commitment to purchase a seat in a

workshop. Whether or not a person actually attends the workshop, your

agency is responsible for payment on the held slot.

3. We do not accept cancellations.  If you provide at least three business

days' notice, you may attend another workshop date. You must attend the alternate workshop within six months of your originally scheduled date.  As an alternative, you may also substitute one staff member for another.  Please contact us to submit the name of substitute.

4. Submit your registration immediately.  Follow with your payment as soon as possible. You may pay with a check, money order, Visa or MasterCard.   Checks should include the workshop number & name of participant and be made payable to, "Good Shepherd Services / HSW."

5. Check-in begins at 9:00 AM. The workshop begins promptly at 9:30 AM. No one will be admitted after 10:00 AM. Please note that there will be no refunds or credit given for latecomers turned away after 10am or for registered participants who do not attend.

6. If a workshop is given in more than one session, participants are expected to attend all sessions and full payment is due if the participant attends any part of the workshop.

7. Certificates of Completion:  Certificates will be given only to

those participants who complete the workshop.

8. When we do not have suffi cient registration to hold a workshop, you will be notifi ed at least 3 business days in advance if you have registered.

9. Once you submit your registration, if you do not receive confirmation within seven business days, please contact us.
